ACA Health Coverage (Form 1095-C)

Use this report to create and print Form 1095-C, which reports employee health coverage information as required by the Affordable Care Act (ACA). RFMS prints directly onto the government form — there’s no need to download it from the IRS website.

Create and Print 1095-C Forms

Step Action Items
1. Open the Report

☐ Go to Accounting > File > Human Resources > Reports > End of Year > ACA Health Coverage (Form 1095-C)

☐ When first opened, complete all required fields in the Employer Information section.

2. Select the Tax Year

☐ From the Tax Year field, choose the correct year. 

☐ Confirm that employees with W-2 earnings for that year appear automatically. 

☐ If needed, update your Tax Tables to the correct year.

3. Verify Employer Information

☐ Under Applicable Large Employer Member (Employer), fill in all identifying information for your business.

☐ This section must be completed before printing is allowed.

4. Locate an Employee ☐ Use Search by Last Name to find a specific employee record.
5. Enter Offers & Coverage Details

☐ Highlight the employee and click Edit

☐ Confirm EDITING appears at the top right of the screen. 

☐ Click Insert or Change to open the Offers & Coverage section. 

☐ Enter Start Month, End Month, Share Cost, Zip Code, Offer Code, and 4980H Code. 

Tip: Refer to IRS 1095-C Instructions for help selecting codes.

6. Add Covered Individuals (if applicable)

☐ Check Covered Individuals if your business offers self-insured health coverage. 

☐ The employee is added automatically; click Insert to list dependents or others covered under the plan.

7. Save Changes ☐ Click Save after adding or editing any information before switching records.
8. Print the Forms

☐ Click Print, then choose Preview or Print Only

☐ Confirm your printer is set to front and back printing (Form 1095-C includes instructions on the reverse). 

☐ Verify all employer information is entered—the form will not print otherwise.

9. Review and Distribute

☐ Review the printed forms for accuracy. 

☐ Provide copies to employees as required by the IRS.
